ParentFit Pty Ltd — Health & Wellness Subscription Service
I / the enrolled participant am voluntarily subscribing to ParentFit Pty Ltd's in-home health and wellness services of my own free will. I understand that I may cancel my subscription at any time in accordance with ParentFit's cancellation policy.
I confirm that all existing medical conditions, injuries, chronic illnesses, recent surgeries and relevant health history of the enrolled participant have been disclosed to ParentFit Pty Ltd prior to commencement of services. It is my responsibility to obtain appropriate medical clearance before the participant begins any fitness components.
Guidance provided by ParentFit professionals — including dieticians, personal trainers, nurses and the ParentFit Buddy — is limited to general health and wellness support. It does not constitute medical advice, clinical diagnosis, or treatment of any medical condition.
Blood tests and diagnostic assessments arranged by ParentFit Pty Ltd are conducted by third-party pathology providers at the participant's home. Results are for general wellness monitoring only and do not constitute a medical diagnosis.
I acknowledge that participation in fitness and physical wellness activities involves inherent risks including physical injury, falls, cardiovascular events or medical emergencies. I voluntarily assume all such risks.
To the fullest extent permitted by law, I release ParentFit Pty Ltd, its directors, employees, contractors and affiliated service providers from any and all claims, losses, injuries or liabilities arising out of the participant's enrolment in ParentFit's subscription services.
I consent to ParentFit Pty Ltd collecting, storing and using the participant's personal and health information to deliver the subscription service, in accordance with ParentFit's Privacy Policy and applicable Australian and Indian privacy legislation.
ParentFit operates on a weekly or monthly subscription basis. A 14-day free trial is available for new subscribers. Cancellation requires a minimum of 7 days written notice and takes effect at the end of the current billing period.
This waiver is governed by the laws of New South Wales, Australia. Any disputes shall first be resolved through good-faith negotiation, then referred to mediation or binding arbitration under applicable Australian frameworks.
